Tru-Care Physical Therapy participates in many health plans and networks.
It is your responsibility to know what your insurance company will cover and if there are any benefit restrictions under your plan.
When you make an appointment with us, we will ask for your insurance information and contact your insurance company to get the details of your coverage. We will review that information and clarify any concerns related to your coverage such as, deductibles, copayments/coinsurance, and allowed physical therapy visits. Each insurance plan is different and benefits can change. All insurances give a disclaimer statement and final payment determination is made when claims are received and processed.

Tru-Care Physical Therapy does accept Workers’ Compensation including Department of Labor as well as motor vehicle accident or other injury claims. If your reason for coming to therapy is related to workers compensation or a motor vehicle accident or third party liability, please print out Attachment A, complete the appropriate section and bring it with your other paperwork to the first appointment.
